Dealer Claims Procedure

Unauthorized Repairs will not be considered for payment The following procedures must be performed before starting repairs

  1. Verify that Vehicle presented for repair is the Vehicle described in the Application (match vehicle identification number).
  2. Verify Holder’s name on Application.
  3. Verify that the Program is in force and has not expired because of mileage or date.
  4. It is the Program Holder’s responsibility to authorize Diagnosis and Teardown.
  5. Verify that you have not performed any previous repairs which may duplicate the present repair.
  6. Determine that the necessary repair is a covered component and/or if the repair is covered under the manufacture’s warranty.
  7. The three C’s are required for authorization. Customer complaint, Cause of breakdown, and Correction needed to repair the vehicle. Please confirm this information with your technician prior to calling.
  8. Estimate cost of repairs. For your convenience, use the worksheet we have provided. Please have all information available when you call the Claims Department. This will help expedite the claim procedure plus provide you with a permanent record that can be attached to your repair order.
  9. Legible original repair order showing breakdown of parts and labor. (Photocopies of accounting copy acceptable.)
  10. Original Sublet bills (towing, machine shop, outside labor, etc.). (Photocopies of accounting copy acceptable.)
  11. Original rental vehicle agreement (if applicable) must show date out and date in, customer’s signature, driver’s license number, and phone number.

Report Claims As Follows:

Provide the following information:

  1. Vehicle Contract Number
  2. Contract Holder’s Full Name
  3. Year, Make, Model and VIN
  4. Repair Order Date and Current Mileage
  5. Repair Order Number
  6. Customer Complaint
  7. Cause of Breakdown
  8. Correction Needed to Repair
  9. Mitchell's Labor Time
  10. Manufacturers List Price on Parts
  11. Cost Plus 10% Mark-up on Sublet
  12. Cost Plus 25% Mark-up on Outside Parts Purchases

You will then be given an authorization number to proceed with the necessary covered repairs.

IMPORTANT: After receiving the authorization number, write the following information clearly on the repair order.

  1. Vehicle Contract Number
  2. Authorization Number
  3. Authorized Amount

How To Make A Claim

In the event of FAILURE, follow this procedure:

  1. Take immediate action to prevent further damage. This ESC will not cover the damage caused by not having the FAILED component repaired as soon as possible.
  2. If YOUR VEHICLE breaks down, return to the Issuing Dealer. If this is not possible, contact ServiceGuard Systems, Inc. at 1-888-464-3774.
  3. Provide the repair facility with a copy of YOUR ESC, and/or YOUR Agreement Number.
  4. Present the repair facility with receipts for required service or Validated Maintenance Log.
  5. Authorize the repair facility to diagnose your vehicle's problem. The repair facility needs to contact ServiceGuard Systems, Inc. with a cause and correction to determine if there is a FAILURE of a covered part. NOTE: disassembly of components/parts may be required on some claims.
  6. Prior to any repair being made, instruct the Service Manager at the repair facility to contact ServiceGuard Systems, Inc. to obtain an authorization for the claim. Any claim for repairs without prior authorization will not be covered except as provided under emergency repairs.
  7. After ServiceGuard Systems, Inc. has been contacted, review with the Service Manager what will be covered by this ESC.
  8. Pay the applicable taxes and DEDUCTIBLE. If the Issuing Dealer retires from business or if YOU move out of his area or if YOU have a claim while traveling, call ServiceGuard Systems, Inc. for instructions.

EMERGENCY REPAIRS - If a FAILURE occurs when ServiceGuard Systems, Inc. offices are closed, YOU should follow the claim procedures above without authorization, and WE will reimburse YOU or the repair facility in accordance with the ESC provisions.

YOU MUST CALL ServiceGuard Systems, Inc.’s OFFICE THE NEXT BUSINESS DAY FROM THE DATE OF REPAIR TO OBTAIN AUTHORIZATION FOR THE REPAIR. ONLY AUTHORIZED CLAIMS WILL BE PAID.
